Operational Aspects of Care Coordination
Care Coordination is critical for meeting population healthcare needs. The Commonwealth Fund website providesarticles that describe operational aspects of care coordination. The article selected for this discussion is Doty, M. M., Tikkanen, R., Shah, A., & Schneider, E. C. (2019). International Survey of Primary Care Physicians in 11 Countries | Commonwealth Fund. Www.commonwealthfund.org. https://www.commonwealthfund.org/publications/journal-article/2019/dec/international-survey-primary-care-physicians-eleven-countries. Strong primary care systems are acknowledged as being helpful in preventing sickness, managing patients’ care across many providers, and lowering healthcare expenditures. To meet the population’s healthcare demands, the primary care system must communicate effectively. Primary care doctors must interact and share information with specialists, patients, social service providers, hospitals, and other care venues.
Effective communication can be implemented in an incremental manner to transition to a value-based environment in different ways. Foremost, utilize various technologies during the delivery of healthcare services. Health information technology helps coordinate care and enhance communication with other healthcare providers and social and community-based services (Doty et al., 2019). Effective communication can assist in the transition to a value-based environment in different ways. In a value-based workplace, activities are done in favor of maximizing overall value through cost savings and better results (Teisberg et al., 2020). In a value-based environment, healthcare providers strive to ensure the best patient health outcome. For a value-based environment, communication between healthcare providers ensures each one has information on what is anticipated of them throughout. Timely access to patient information allows healthcare providers to provide quality healthcare services that will help meet their needs (Adane et al., 2019). Patients have optimum health outcomes following a value-based environment, enhancing healthcare providers’ reimbursement in healthcare settings. Primary care physicians are highly motivated by increased reimbursement, engaging in activities that would lead to optimal patient outcomes.
