Multiple organ systems can contribute to pelvic pain. Gastrointestinal, gynecological, genitourinary, and musculoskeletal systems must be considered in patients with this symptom. This is a joint presentation in primary care. Evaluation can be challenging because of a broad differential diagnosis, and any associated signs and symptoms are nonspecific (Bower, 2020).
Common diagnoses of acute pelvic pain in women of childbearing years with acute pelvic pain are idiopathic pelvic pain, pelvic inflammatory disease, acute appendicitis, ovarian cysts, ectopic pregnancy, and endometriosis (Hecht et al., 2019). Women of reproductive age should take a pregnancy test. If pregnant and early in pregnancy, transvaginal ultrasonography and beta-human chorionic gonadotropin level can help determine ectopic pregnancy or spontaneous abortion if the woman is not pregnant. A referral to a surgeon is needed if the patient has an ectopic pregnancy. An ultrasound when ovarian pathology is suspected or CT depending on the possible diagnosis. A postmenopausal woman with acute pelvic pain cancer, endometriosis, and fibroids (Hecht et al., 2019).
Also, gastrointestinal such as appendicitis and diverticulitis must be considered. Urinary ureteral obstruction, complicated UTI, and sepsis must also be considered(Hecht et al., 2019). Referral to an oncologist for cancer diagnoses, Urology for ureteral obstruction, and surgeon for appendicitis.
It is essential to obtain a focused history by describing the pain, date of LMP, changes in menstrual pattern, presence of vaginal discharge, dysuria, hematuria, nausea, recent surgery, medications, and allergies(UpToDate, n.d.). Asking if the patient is sexually active, pain related to intercourse or pain related to pregnancy.
